This is a clean contact form with social links and email notification. It is resizeable and it can show up to 3 textfields in the description + an image.
Very easy to customize through the xml file.

Note that in the preview the e-mail is not actually sent. It is just a simulated response from PHP.
In the PHP you can configure the message body and also the message the visitor will receive after sending you the mail.
The code is commented and the help file is provided (as well as the free icons used)
